Accidents are unplanned and unintentional events that result in harm or loss to personnel, property, product or nearly anything that has some inherent value. Accident are rarely simple and almost never result from single cause. Most accidents involve multiple, interrelated causal factors. Accidents can occur whenever significant deficiencies, oversights, errors, omissions, or unanticipated changes are present. Any one of these conditions can be a precursor for an accident; the only uncertainties are when the accident will occur and how severe its consequences will be.

To conduct a complete accident/incident investigation, the factors contributing to an accident, as well as means to prevent accidents, must be clearly understood. Management prevents or mitigates accidents identifying and implementing the appropriate controls and barriers. Accidents occur when one or more barrier of a work system, including procedures, standards, and requirements intended to control the actions of workers, fail to perform as intended. The barriers may not exist, may not be adhered to, or simply may not be comprehensive enough to be effective. Personal performance and environmental factors may also reduce protection.

Understanding how to prevent or control accidents requires an understanding of the sequence of events leading to an accident in order to identify and implement countermeasures that contain risks.

Objective:

The purpose of this one day program is to provide those responsible for conducting accident investigations with practical detailed instruction on conducting these investigations. The program provides in-depth guidance, as well as specific tools and techniques, that will facilitate the investigations process.

Course Leader
Dr. C.J. Krishna, PhD
Dr. C.J. Krishna was a trade union activist for last 30 years and his interest was in the field of industrial relations and occupational safety and health & workmen compensation. From 1992 to 1994 Krishna, served the OSHA Committee headed by the Minister of Human Resources. In 1989 Krishna studied at the Sheffield University (UK) for the Post Graduate on ILO Fellowship and majored in Safety and Health and his thesis on ‘Developing and Selection of Tools for Safety Training’ at British Oxygen, Sheffield, was well received by BOC and the university. In 2004 Krishna was awarded the Doctor of Philosophy (PhD UNC) and his 300 page thesis for his PhD ‘Selection of Materials and Developing Tools For Training in Occupational Safety and Health in the Malaysian Perspective’ was well received by the university. Dr. Krishna was a panel member of industrial court and also a registered labour representative in Malaysia. Dr. Krishna teaches in colleges for MBA and supervises PhD students. Dr. Krishna has been the Vice President of the Malaysian Association of Human Resources Consultants and the Secretary General of Malaysian Occupational Safety and Health Professional’s Association. Dr. Krishna has in record 5 principle authorship in Training & Development, Occupational Safety and Health and SOCSO (SOCIAL SECURITY) published by the Commercial Clearing House (CCH), Industrial Relations and Compensation and Benefits. Dr. Krishna specializes in Accident Compensation and Performing Monitoring
